Detailed description:

The rules for including <assert.h> are partly in 7.1.2 and partly unstated.
They should be stated more clearly in 7.2. Add to the end of 7.2p1:

    The /assert/ macro is redefined according to the current state of
    /NDEBUG/ each time that /<assert.h>/ is included.
